Improving Awareness of Safe Sleeping Practices for Babies on a Mother and Baby Psychiatric Unit

Introduction
SIDS is the sudden, unexpected and unexplained death of a baby. Safe sleeping practices can help to reduce a baby’s risk of SIDS. At the Coombe Wood Mother & Baby Unit (MBU), it was found that many patients were opting to co-sleep with their babies which contradicts safe sleeping guidelines.
ObjectivesTo improve patient awareness of the condition SIDS and to implement an interactive training session improving awareness of safe sleeping practices for babies. To improve patients’ confidence in implementing safe sleeping practices for their babies; thus reducing the risk of SIDS occurring.
MethodsThe Lullaby Trust™ is a charity that raises awareness of SIDS and provides expert advice on safe sleep for babies. An interactive training session for patients was organised by incorporating published materials from The Lullaby Trust™, facilitated by medical and occupational therapy staff on the MBU. The participants filled out a pre-training and post-training questionnaire to test the effectiveness and quality of the training session.
ResultsThe participants’ average level of confidence in knowing and applying safe sleeping practices for their babies doubled following the training session (from 2.3→4.8 and 2.6→5 respectively, with 5 meaning “Very Confident.”) The average level of knowledge of SIDS also increased from 1.6→4.4 (with 5 meaning “A Lot” of Knowledge.)
ConclusionsWe were surprised at the low level of knowledge and confidence the patients had regarding safe sleeping practices for their babies. This project shows how interactive, ward-based training can be an effective way to engage and stimulate patients into improving the safety of their baby care.

Human campylobacteriosis exhibits a distinctive seasonality in temperate regions. This paper aims to identify the origins of this seasonality. Clinical isolates [typed by multi-locus sequence typing (MLST)] and epidemiological data were collected from Scotland. Young rural children were found to have an increased burden of disease in the late spring due to strains of non-chicken origin (e.g. ruminant and wild bird strains from environmental sources). In contrast the adult population had an extended summer peak associated with chicken strains. Travel abroad and UK mainland travel were associated with up to 17% and 18% of cases, respectively. International strains were associated with chicken, had a higher diversity than indigenous strains and a different spectrum of MLST types representative of these countries. Integrating empirical epidemiology and molecular subtyping can successfully elucidate the seasonal components of human campylobacteriosis. The findings will enable public health officials to focus strategies to reduce the disease burden.
Guillotine versus dissection tonsillectomy: randomised, controlled trial

Objective:
This trial aimed to compare the guillotine technique of tonsillectomy with ‘cold steel’ dissection, the current ‘gold standard’.
Design:A single centre, randomised, controlled trial.
Methods:One hundred children aged 3 to 11 years who were listed for bilateral tonsillectomy were recruited. Patients had one tonsil removed by each technique, and were blinded to the side. The operative time, intra-operative blood loss, haemostasis requirement and post-operative pain scores were recorded and compared.
Results:Operative time and intra-operative blood loss were both significantly less for the guillotine technique (p < 0.001) and there was a significantly reduced haemostasis requirement (p < 0.001). Pain was also less on the guillotine side (p < 0.001). There were no tonsillar remnants or palatal trauma for either technique. There was no significant difference between techniques in the frequency of secondary haemorrhage.
Conclusion:This study provides level Ib evidence that guillotine tonsillectomy in children with mobile tonsils is an effective and time-efficient procedure which produces less intra-operative blood loss and post-operative pain than cold steel dissection.

Fast Detection of Perkinsus Marinus, a Prevalent Pathogen of Oysters and Clams from Sea Waters

Antibody-functionalized, Au-gated AlGaN/GaN high electron mobility transistors (HEMTs) were used to detect Perkinsus marinus. The antibody was anchored to the gate area through immobilized thioglycolic acid. The AlGaN/GaN HEMT were grown by a molecular beam epitaxy system (MBE) on sapphire substrates. Infected sea waters were taken from the tanks in which Tridacna crocea infected with P. marinus were living and dead. The AlGaN/GaN HEMT showed a rapid response of drain-source current in less than 5 seconds when the infected sea waters were added to the antibody-immobilized surface. The recyclability of the sensors with wash buffers between measurements was also explored. These results clearly demonstrate the promise of field-deployable electronic biological sensors based on AlGaN/GaN HEMTs for Perkinsus marinus detection.
Laryngeal squamous cell carcinoma presenting as a prelaryngeal, air-filled structure (pseudolaryngocoele)

Objective:
We report a unique case of a squamous cell carcinoma of the larynx presenting as a large pseudolaryngocoele, arising through a thyroid cartilage defect.
Method:Case report and review of the literature.
Case report:A 47-year-old man presented with a two-month history of hoarseness and a large, midline neck swelling. Endoscopic examination revealed a transglottic carcinoma involving the anterior commissure. Fine needle aspiration of the neck mass showed it to be an air-filled structure which transiently collapsed but refilled within minutes. Subsequent computed tomography scanning and histopathological examination revealed that the air-filled mass was created by a defect in the thyroid cartilage, with formation of a pseudolaryngocoele.
Conclusions:The anatomy of the anterior commissure region and its effect on the spread of laryngeal carcinoma is reviewed in order to explain the pathophysiology of this unusual presentation. We highlight the need for a high index of suspicion of malignancy if a laryngocoele or pseudolaryngocoele is detected clinically.
Status of Nitride Based Light Emitting and Laser Diodes on SiC

Single crystal thin films with compositions from the A1N-InN-GaN system were grown via metal-organic chemical vapor deposition (MOCVD) on single crystal 6H-SiC substrates. Blue light emitting (LED) and laser diode (LD) structures were fabricated. The conducting buffer layer LEDs employed an AlGaN buffer layer which provides a conduction path between SiC and the active device region. The external quantum efficiency of the LEDs was 3% at 20 mA- 3.6V and peak emission wavelength of 430 nm. Violet and blue LDs were fabricated and consisted of an 8-well InGaN/GaN multiple quantum well (MQW) active region in a separate confinement heterostructure (SCH) design. Lasing was obtained both on structures using an insulating buffer layer, and also on structures using a conducting buffer layer. The resulting lasers operated at room temperature using pulsed and continuous wave operation with an emission wavelength of 404-435 rim. The lowest threshold current density obtained for lasing was 11 kA/cm2.
The First Nitride Laser Diode on Silicon Carbide

This paper reports the events at NCSU leading up to and including those of June 5, 1997 which produced the first demonstration of a nitride laser diode on silicon carbide – and the very first nitride laser demonstration outside of Japan. All of the laser diode samples tested at NCSU were designed, grown, and fabricated into cleaved cavity test structures at Cree Research. Laser testing at NCSU consisted of spectral emission versus current measurements, light output power versus current (L-I) measurement, and light output polarization measurements versus current. The first successful laser on silicon carbide emitted at 402.6 nm. Subsequently, lasers displaying outputs ranging from 402.6 to 430.2 have been successfully tested at NCSU.
Iodine metabolism and thyroid hormone relationships in growing sheep fed on kale (Brassica oleracea) and ryegrass (Lolium perenne)–clover (Trifolium repens) fresh-forage diets

1. Kale (Brassica oleracea) and ryegrass (Lolium perenne)–clover (Trifolium repens) pasture, grown under similar soil conditions, were grazed in the vegetative state by growing lambs of 23·6 kg initial live weight for 24 weeks. The kale and pasture contained respectively 20 and 270 μg iodine/kg dry matter (DM). The kale also contained 8 μmol total glucosinolates/g DM and 11·5 g S-methyl-L-cysteine sulphoxide (SMCO)/kg DM, both of which were nondetectable in the pasture diet.
2. Intramuscular injections of 1 (475 mg) were given during weeks 1 and 12 to half the forty-eight lambs grazing each forage. Wool growth, live-weight gain and cytochrome oxidase (EC 1.9.3.1) activity of biopsied hind-limb muscle were measured at 6-week intervals. Jugular blood samples were removed every 6 weeks for the determination of haematological factors and serum thyroid hormone concentrations. All animals were slaughtered at the end of the experiment and thyroid weight, thyroid I content, and the weight and cytochrome oxidase activity of heart muscle determined.
3. Serum concentrations of thyroxine (T4) increased from 20 to 48 nmol/l during the 24 weeks that control lambs grazed ryegrass-clover pasture. I supplementation increased the concentration and total amount of I in the thyroid gland and increased serum T4 concentration, but did not affect any other values measured in the lambs grazing the pasture herbage. Serum concentrations of triiodothyronine (T3) were stable at 2 nmol/l for both groups.
4. Control lambs grazing kale for 24 weeks showed marked thyroid enlargement and depletion of thyroid I. By week 6, serum T4 and T3 concentrations had declined to 2–5 nmol/l and 1 nmol/l respectively and were stable at these values for the remainder of the experiment. I supplementation eliminated the thyroid depletion of this element, caused serum T4 concentration to rise and stabilize at 90 nmol/l by week 18, and T3 concentration to stabilize at 2 nmol/l by week 6. From week 6 onwards, wool growth was increased 13% by I supplementation, whereas empty body growth was unaffected.
5. Lambs grazing kale developed haemolytic anaemia, due to rumen fermentation of SMCO. I supplementation enabled the lambs to resist the anaemia better by increasing erythrocyte reduced gluthathione (GSH) content. Relative to pasture-fed animals, lambs grazing kale and supplemented with I showed increased heart muscle weight and cytochrome oxidase activity. This represented a compensatory mechanism for the reduced blood oxygen-carrying capacity caused by the anaemia. I-deficient (i.e. control) lambs grazing kale showed reduced cytochrome oxidase activity in both heart and hind-limb muscle.
6. The findings are in accord with T3 having a greater biological potency than T4 for regulating rates of body and wool growth. Increases in heart weight, heart cytochrome oxidase content and erythrocyte GSH content of kale-fed lambs were, however, associated with elevation in serum T4 and not T3 concentration.
7. I requirements of growing sheep and cattle consuming the pasture diets are discussed. Because of its better relationship to production traits, it is considered that requirements should be based on the ability to maintain T3 rather than T4 concentrations. On this basis, requirements could be met by diets containing 180–270 μg I/kg DM.
